TATTOO TERMS AND CONDITIONS

By getting a tattoo at Stone Circle you agree to:

• Release Stone Circle Pty Ltd, ACN 333333333 its employees, contractors and agents and hold them harmless from all liabilities, including, but not limited to death, injury, medical problems and other health related issues and against all sums of money, actions, suits, causes of action, claims, proceedings, accounts, demands, costs and expenses arising out of or in any way connected with the services received by me
• Sign a release form and agree to be legally bound by the information given.
• Read the release form and confirm that all the information given by me is true and correct.
• Confirm I am 18 years of age or older. (Proof of I.D. is to be provided).
• I understand my I.D. will be photocopied and a copy will be kept with this release form for future proof of age and will be produced if required by VIC Fair Trading..
